Student Health

On enrolment, you will be asked to provide your child’s health information to help us meet your child’s health needs.

You will be asked to:

  • Complete a Health Care Summary form with details about health care needs and information to use in a medical emergency

Provide evidence of Immunisation:

  • Provide your child’s Australian Immunisation Register (AIR) Immunisation History Statement, no more than two months old, with a status of ‘up to date’; or
  • an AIR Immunisation History Statement, no more than two months old, showing the status is ‘not up to date’ and an approved catch-up schedule; or
  • a valid immunisation certificate issued or declared by the Chief Health Officer.

You can access your Childs Australian Immunisation Register (AIR) Immunisation History Statement by:

  • phoning the Australian Immunisation Register enquiries line on 1800 653 809
  • visiting Medicare Online through the MyGov website
  • using the Express Plus Medicare app
  • visiting a Medicare or Centrelink office.

If your child needs to be given medication during school hours, you will need to provide:

  • Medication that is labelled with your child’s name. This must be provided in the original packaging with the expiry date visible
  • Completed Administration of Medication form, which can be obtained from the school
  • Diagnosis letter, if relevant

The school is unable to administer any medication without these forms being complete.
